Documents Required for Bihar SHS Document Verification 2026

Candidates must carry the ORIGINAL documents along with one set of self-attested photocopies.

Essential Documents Checklist:

1. Educational Certificates: 10th, 12th, Diploma in Optometry / Ophthalmic Assistant Certificate (2 years), provisional certificates.
2. Identity Proof Documents: Aadhar, PAN, Voter ID, Passport (if available), Driving License (if available).
3. Category/Reservation Certificates: Caste (SC/ST/OBC), EWS, Creamy Layer (OBC), PwD certificate (40% and above).
4. Other Mandatory Documents: CBT Admit Card, Application form printout, Date of Birth Certificate, photographs (10-15 copies), Domicile Certificate (if required).

Important: Carry documents in a TRANSPARENT FILE FOLDER. Self-attest photocopies. Originals may be returned after verification.

Important Instructions for Document Verification

1. Arrive at least 30 minutes before the scheduled time. Late entries may be rejected.
2. Dress code: Formal attire; avoid casual clothes, shorts, or flip-flops.
3. Organize documents in the prescribed order.
4. Self-attest all photocopies. Unsigned copies may be rejected.
5. Do not send originals through any representative; attend in person.
6. Mobile phones and electronic devices may not be allowed inside the verification hall.
7. Carry at least 2 government-issued photo IDs.
8. If there are discrepancies in names or DOB, carry an affidavit.
9. If final certificates are not available, bring provisional certificates with an undertaking to submit originals later.
